RE: Firefighter Award Program
 

                The Utah State Firemen’s Association, in cooperation with the Utah State Fire Marshal,  the Utah Fire & Rescue Academy,  Utah Fire Chiefs Association and American LaFrance, is extremely pleased to announce the Utah Firefighter of the Year Awards.  The purpose of this Award is to honor Utah’s finest fire service personnel in a number of different categories.  Specific information on the categories, eligibility and the process for nomination is attached.  Any person may nominate firefighters or particular fire organizations for these awards, but the local fire chief must verify the nomination information by signing the attached form.  Please make copies, post them and distribute them.  Please convey complete information on the form or additional sheets, as we will only be able to consider the information included with the submitted application.

                The selection will be made in late March with the actual presentation to be made at the Utah State Firemen’s Association Convention.  The winners will also be honored at the Utah State Museum of Fire Service History and in a statewide press release.

                One nominee (who must be a volunteer) will also be forwarded to the National Volunteer Fire Council for consideration in the National Volunteer Firefighter of the Year Award program.

UTAH FIREFIGHTER OF THE YEAR AWARDS

CRITERIA

 I.  ELIGIBILITY

 Any paid, call, or volunteer Utah Firefighters may be nominated by any person for any of the awards, but to be awarded Firefighter of the Year and Fire Company Recognition Awards, the successful nominee must be a member of a department chartered by the Utah State Firemen’s Association.  Nominations may be initiated by anyone and must be approved and signed by the head of the local fire department and must be of performance of the last calendar year.  Nominations must be received by the Selection Committee by March 10th.  All applications must be accompanied by a photograph of the nominee(s).  Other background information may be submitted with the nomination form and will be considered with it.

II.  AWARD CRITERIA

Nominations may be made to recognize individuals for performance in any of the following areas:

a)            MEDAL OF VALOR

                Awarded to an individual or individuals as the highest recognition of bravery.

b)            MERITORIOUS AWARD FOR COMMUNITY SERVICE, INDIVIDUAL

                Awarded to an individual for exemplary community service programs and/or actions.

c)           MERITORIOUS AWARD FOR COMMUNITY SERVICE, TEAM

                Awarded to a team of individuals for exemplary community programs and/or actions.

d)            FIRE COMPANY RECOGNITION

                Awarded ONLY for an Association Member group or unit’s performance.  Can go to a department or part thereof.

e)            FIREFIGHTER OF THE YEAR

                Awarded ONLY to an Association Member for outstanding service in any of the above categories.
